$100/month +
$100 enrollment fee per person or $200 maximum enrollment per family when enrolling multiple members under one guarantor

All the benefits of the Essential membership +
Evidence-based perimenopause/menopause management, with HRT if desired
Weight loss support through intensive lifestyle management, and thoughtful and responsible GLP-1 medication management, if desired
Dedicated lifestyle medicine intake and monthly follow ups focused on wellness/lifestyle goals like weight management, optimized nutrition, reduction of risky substance use, gut health, sleep, stress management, etc.
Exclusive access to a personalized in-home pantry set up, crafted meal planning, and grocery store excursion with Dr. Pfenning
Access to all upcoming lifestyle medicine initiatives like Dr. Pfenning’s upcoming culinary medicine cooking classes at local teaching kitchens

$450 per 2-3 hour home visit/grocery store excursion

When you click to enroll, you will be taken to the practice EMR, SigmaMD. This will serve as your patient portal and interface with the practice.
You can cancel your membership with 30-days notice. However, should you request to rejoin Vital Roots, you’ll be charged a $350 re-enrollment fee.
Dr. Pfenning does not prescribe narcotics (tramadol, Oxycodone, Percocet, etc.) for chronic, daily pain.
Dr. Pfenning does not prescribe benzodiazepines (Xanax, Klonopin, alprazolam, clonazepam, etc.) for chronic, daily use (unless for taper off chronic benzo regimen with goal of complete stoppage.)
Dr. Pfenning does not prescribe z-drugs (zolpidem, eszopiclone, Ambien, Lunesta, etc.) for chronic, nightly use.
